This form was designed for patients leaving the hospital after an outpatient surgery. The doctor needed to provide clear, easy to read instructions for his patients so that they could take care of themselves at home and have the most important questions answered right away. They wanted something that would make it easy for the patients to contact them if they had and complications for concerns but also be able to know exactly what they needed to do to heal at home.

Wedding shower invite designed to fit with the Bride’s flower and color scheme for her wedding. She wanted circles, flowing script and sunset colors.
